Prevent Medical Error is committed to ensuring your right to medical privacy. We do not sell or disseminate your Private Health Information (PHI) or your name and contact information. PME is not responsible or liable for interceptions of and/or patient or provider disclosure of Private Health Information. Prevent Medical Error shall not be liable for the transfer of any personal identification information resulting from loss or distribution of data, the delineation or corruption of storage media, power failures, natural phenomena, riots, acts of vandalism, sabotage, terrorism or any other event beyond PME's control.
Prevent Medical Error may form strategic alliances with another party to provide specific services. When you agree to these services, PME will share names, or other contact information that is necessary for the other party to provide these services. Per Prevent Medical Error's contractual arrangements with parties, these parties are not allowed to use personally identifiable information except for the explicit purpose of providing these services.
Under certain limited circumstance, Prevent Medical Error may relay patient safety information. This information does not include any personal identifying information. Examples may include but not be limited to health reporting agencies or to law enforcement agencies to enforce laws and third party rights, such as intellectual property rights and laws relating to fraud. In response to a verified request by law enforcement or other government agencies related to alledged illegal activity, or other activity that may expose PME or you to legal liability, PME will disclose your Private Information as PME, in its sole discretion, sees fit.
You may choose to contact us, but You agree not to relay any material the content of which (i) is defamatory, libelous, obscene, indecent, abusive, threatening to others, or in violation of any law; or (ii) infringes the copyright, trademark right, or other intellectual property right of any third party (iii) is for organized commercial or political activity.
